Transitional Care Clinic Nurse Practitioner
A Transitional Care Nurse Practitioner is responsible for coordinating and managing patient care during transitions between care settings, such as from hospital to home or post-acute facilities. This role focuses on reducing readmissions by providing comprehensive assessments, medication reconciliation, patient and caregiver education, and timely follow-up care. The nurse practitioner collaborates closely with physicians, care management, and community providers to ensure continuity of care. Strong clinical judgment, communication skills, and a patient-centered approach are essential to improving outcomes and supporting high-quality transitions of care.
